Permission for Prescription Medication Administration
If your child requires the administration of prescription medication(s) during the school day, it is required that the Permission for Prescription Medication form be completed and signed by both a parent/guardian AND the prescribing health care provider (physician, physician assistant or nurse practioner). School staff will not administer prescription medication without this completed form. This form must be completed for each medication to be given and must be completed every school year.
Once the form is completed, it can be dropped by the main office, mailed to 104 W. 4th, PO Box 98, Cunningham, KS 67035, or faxed to 620-298-2562.
